Technically, it's more like the Training Room goes haywire everytime we, the audience, sees them use it. One presumes that they use it all the time off-screen where nothing goes wrong. I would think the holodeck is fairly similar. You often hear people wondering why any one would ever use the stupid thing, since it's always breaking down or creating super villains, but the truth is that most of the time, it probably works flawlessly, and the only time there are problems are when the show focuses on it.
